VAERS
VAERS stands for Vaccine Adverse Event Reporting System. Per Wikipedia:
VAERS is co-managed by the U.S. Centers for Disease Control and Prevention (CDC) and the Food and Drug Administration (FDA). VAERS is a post-marketing surveillance program, collecting information about adverse events (possible harmful side effects) that occur after administration of vaccines
The use of the VAERS system is voluntary, and is an extremely difficult and time-consuming app (it takes about a half-hour to make one report on one incident) so it is not used to report the majority of vaccine injuries. It’s estimated that only about one in 40 - or even as few as one in 100 vaccine incidents - are reported in VAERS, making the reporting of vaccine injuries overall significantly underreported. And once reported it’s taking a year or more for the VAERS people to investigate the open cases.
Although the CDC states that they are monitoring vaccine injuries, the CDC has never commented on the VAERS numbers or statistics.

Not only is the CDC not following up on VAERS data, they are deleting VAERS data from the database without comment at an alarming rate. For those of you who understand data management, deleting the report is an unacceptable method of adjudicating the reports. Instead, the reports should be flagged in the system with a reason that the report should no longer be counted:
